Clinical Social Worker, Triage Team (Remote - US only)
Candidates must hold an active, unrestricted independent clinical social work license in their current US state and be willing to obtain additional state licenses for cross-state practice. This is a remote clinical social work role on the triage team of a value-based oncology care company that works with health plans, providers and employers to support people with cancer and their caregivers. The clinician acts as the primary intake point for escalated behavioral health and complex social needs cases, completing time-sensitive psychosocial and safety assessments by phone or video. The shift runs 11:30 AM to 8:00 PM Eastern Time on a four 10-hour-day schedule.
What you'll do
- Serve as primary intake clinician for escalated behavioral health and advanced social needs consults, completing psychosocial and safety/risk assessments by phone or video.
- Deliver brief interventions for stabilization and safety planning, including de-escalation, suicide risk management and immediate safety planning.
- Conduct comprehensive psychosocial assessments covering anxiety, depression, grief, caregiver strain, financial hardship, advance care planning and existential distress, and provide therapeutic interventions.
- Decide escalation pathways for behavioral health crises and social safety concerns under the organization's clinical escalation procedures, and notify the appropriate clinical leads.
- Coordinate with nurses, care partners, providers and community organizations to remove barriers to care, track referrals and ensure continuity until a stable plan is in place.
- Document assessments, safety plans, referrals and escalations to organizational standards, and provide concise clinical consultation to the care team.
What you need
- Master's degree in Social Work (MSW) from a CSWE-accredited program.
- Valid, active, unrestricted independent clinical social work license (such as LCSW or LISW) in your current US state.
- At least 3 years of clinical social work experience in a healthcare setting; crisis intervention and behavioral health experience strongly preferred.
- Proven skill in risk assessment and crisis management (suicidality, homicidality, psychosis) and strong telehealth and phone clinical skills.
- Sound clinical judgment for time-sensitive decisions while working remotely.
- Available for four 10-hour days including Monday, Friday, Saturday and one day of your choice. Saturday availability is temporary and expected through September.
- Oncology or palliative social work experience and OSW-C or APHSW-C certification are preferred but not required.
