Care Navigator (Remote, US - Contract)
Pay: $43.50/hr - $43.50/hr (hourly rate). Open to candidates based in the US; the role works with US hospital partners and US insurance and community resources. A behavioral health technology company is launching a new care navigation and triage program. It helps patients leaving emergency departments, inpatient psychiatric units and partial hospitalization programs connect quickly and safely to the right mental health care. The first weeks after discharge carry high risk, so timely follow-up can make a real difference. You will be the main point of contact for hospital partners and patients, working alongside a licensed Clinical Assessor, and you will help shape a program that is still being built.
What you'll do
- Receive referrals from partner hospitals and serve as the main contact for discharge planners and care teams.
- Route each referral according to set guidelines and a triage protocol, either to a provider directly or to a Clinical Assessor for evaluation.
- Schedule patients with the Clinical Assessor and book them into the recommended level of care: a provider, a specialty program or a community resource.
- Follow up to confirm patients attended, and act quickly when an appointment falls through.
- Help patients with practical barriers such as transportation, food, and insurance or benefits questions, and coordinate between their providers.
- Build and maintain local referral resource lists in each market, and escalate to clinical staff immediately when a patient may be in crisis.
What you need
- At least 2 years of experience in behavioral health care coordination, case management, patient navigation or hospital discharge planning.
- Comfortable working with recently discharged patients who may be in distress, and knowing when to bring in clinical support.
- Highly organized, able to manage a high volume of referrals and follow-ups on tight timelines without losing details.
- Clear, warm communicator with patients, hospital staff and providers.
- Able to set up or already hold an LLC or S-Corp, as this is a contract role.
- Based in the United States. Familiarity with insurance and community resources in Missouri or Virginia is a plus.
Applicants must be based in: United States.
