Complex Care Physician - Oncology Navigation (Remote - US only)

All patient care is delivered virtually by video, phone, text, or a care platform. A fast-growing, technology-forward company that supports people with cancer and other serious chronic conditions is hiring a physician for its complex care team. You will work in a fully virtual, interdisciplinary model alongside community health workers, oncology nurses, social workers, and nurse practitioners. The role focuses on helping members address both medical and social needs while keeping their care aligned with their personal goals.

What you'll do

• Lead interdisciplinary case conferences and offer clinical guidance on complex or advanced cases.
• Provide virtual urgent and same-day visits, using sound clinical judgment for assessment, triage, and next steps.
• Conduct virtual post-discharge visits that follow evidence-based guidelines, sometimes with a field-based community health worker joining the telemedicine visit.
• Build and adjust individualized care plans with the care team as members' needs change.
• Coordinate with external oncology and primary care providers to reduce fragmentation across care settings, and contribute to clinical quality through education, feedback, and documentation.

What you need

• Board certification in internal medicine or family medicine.
• Unrestricted U.S. medical license, and willingness to obtain additional state licenses as needed. Must be based in the United States.
• Demonstrated experience delivering virtual or telehealth care, with strong communication and documentation skills.
• Experience managing medically complex patients and supporting shared decision-making within interdisciplinary teams.
• Comfort using video visits, electronic health records, and other digital tools in daily practice.
• Geriatrics, palliative care, or oncology fellowship training, prior startup or value-based care experience, and interest in data-driven improvement are a plus.
