Palliative Oncology Nurse Navigator (Remote - US only)

Pay: $46.35/hr - $46.35/hr (hourly rate). Shift hours are 11:30am - 8:00pm Eastern Time. This is a remote clinical role with a value-based oncology care company that supports people with cancer and their caregivers. You will serve as a palliative care nurse navigator, holding conversations about goals, values, advance care planning and palliative care by phone, email and video. The role combines hands-on member support with coordination across the care team, payers and providers. Your work centers on symptom assessment, longitudinal support and, when appropriate, end-of-life discussions.

What you'll do

• Complete thorough symptom assessments and address clinical concerns for members with serious illness.
• Hold goals-of-care, advance care planning and end-of-life conversations with members and their care networks.
• Support members through case conferences and collaborate on targeted support plans with nursing leaders and medical directors.
• Coordinate with payers, providers and non-clinical team members on needs such as food access and transportation.
• Manage urgent clinical escalations and provide clinical consultation as needed.
• Keep clear, accurate documentation and follow team policies and clinical protocols.

What you need

• Active, unrestricted compact Registered Nurse (RN) license. Licensure in New York, Illinois or Michigan is preferred.
• ADN or BSN required; BSN preferred.
• At least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice caring for oncology patients and their caregivers.
• OCN certification required at hire, or a commitment to obtain it within 2 years of hire. Certifications such as CHPN, AOCN, AOCNS or CCM are a plus.
• Must be located within the lower 48 United States due to contractual access requirements for protected health information.
• Fluency in written and spoken Spanish is preferred.
