Pharmacy Care Coordinator (Remote)

A fully remote telephone-based role supporting a healthcare pharmacy programme that connects patients with a specialised mail-order pharmacy offering pre-sorted multi-dose medication packs, refill management and periodic care check-ins. The organisation works with partner healthcare practices to support patients living with chronic kidney disease and other complex long-term conditions. The role suits a warm, organised communicator with healthcare or patient-support experience who enjoys building trust over the phone, explaining services clearly and helping people stay consistent with their medicines. Working hours are aligned to the Eastern time zone.

What you'll do

• Conduct high-volume outbound calls to introduce the pharmacy service and guide patients through enrolment
• Explain how multi-dose packs, home delivery and refill coordination work using approved talking points
• Obtain and document patient consent by phone and capture information needed for enrolment
• Update patient records in the electronic health record, including preferred pharmacy and electronic prescriptions, based on patient decisions
• Manage outreach queues and follow-up attempts, prioritising by urgency
• Keep accurate records of call activity and outcomes while protecting patient privacy under HIPAA

What you need

• Current Medical Assistant certification (CMA or RMA)
• Experience in telephone, call-centre or high-volume outbound outreach
• Experience in healthcare, pharmacy, care coordination or patient support
• Proficiency with electronic health records and Microsoft Office including Outlook and Teams
• Working knowledge of HIPAA and strong attention to detail
• Home internet of at least 25 Mbps download and 10 Mbps upload, and a dedicated quiet workspace
